Base pay covers every hour at straight time. The overtime premium is the extra half (0.5 × $45 = $22.50) on each overtime hour. Together that's the familiar $67.50 an hour for overtime, shown in two lines so the breakdown adds up.
How nurse overtime is calculated
- Find your overtime hours. Paid hours past the daily threshold (if your employer has one), or past the weekly threshold once earlier hours this week are counted — whichever gives more. An hour is never counted twice.
- Find the rate overtime is paid on. Your base rate, or your base rate blended with hourly differentials.
- Add the premium. Overtime hours × (multiplier − 1) × that rate. Double time works the same way with its own threshold and multiplier.
Why differentials matter: on a 12.5-hour paid shift at $50 with a $5 night differential and daily overtime after 12, the overtime premium is $13.75 when the differential is blended into the overtime rate, and $12.50 when it isn't. Under the federal Fair Labor Standards Act, shift differentials are generally part of the "regular rate" overtime is based on (29 CFR 778.207), which is why blending is the default here. Your employer's payroll decides what actually happens.

Assumptions and limits
- Thresholds and multipliers are the ones you enter. Which overtime rules apply to you depends on your employer, contract, union agreement and state law.
- "Hours already worked this week" should be paid hours in the same work week — not PTO or on-call hours unless your employer counts them.
- Some hospitals use a different overtime arrangement for nurses (for example, 8-and-80 pay periods). Enter the thresholds that match your employer's actual policy.
Common overtime situations
- Three 12s, then a pickup shift
- Weekly threshold 40, hours already worked 36. The first 4 hours of the pickup are regular, the rest overtime.
- Daily overtime after 8
- Set daily overtime to 8. A 12-hour paid shift has 4 overtime hours, even in a short week.
- Overtime only after 12 hours
- Set daily overtime to 12 and clear the weekly threshold if your employer doesn't use one.
- Double time after 12
- Set double time after 12. Hours past 12 are paid at 2× instead of 1.5×, not both.
Frequently asked questions
Is nurse overtime always time-and-a-half?
1.5× is the most common multiplier, but contracts and union agreements can pay more. Enter your own multiplier.
Do shift differentials count toward overtime?
They can raise the rate overtime is paid on. Use the "Include differentials in the overtime rate" switch to see both ways.
Does an unpaid meal break count toward overtime?
No. Only paid hours count toward overtime thresholds, so an unpaid break reduces the hours that can become overtime.
What if I have daily and weekly overtime?
Enter both. The calculator uses whichever gives more overtime hours for the shift, and never pays an hour two premiums.
